Endoscopic Ultrasound (EUS) is an advanced procedure that combines endoscopy and ultrasound to obtain detailed images of the digestive tract and nearby organs.

What is EUS?

Endoscopic Ultrasound (EUS) is a specialized procedure that combines:

  • Endoscopy → a thin flexible tube with camera
  • Ultrasound → sound waves to create detailed images

It helps doctors see the digestive tract and nearby organs from very close range, giving much clearer images than a normal abdominal ultrasound.

Which Organs Can Be Examined?

EUS can evaluate:

  • Food pipe (esophagus)
  • Stomach
  • Duodenum/small intestine
  • Rectum & colon
  • Pancreas
  • Gallbladder
  • Bile ducts
  • Liver
  • Lymph nodes
  • Mediastinum/chest structures

Pancreatic Diseases

  • Pancreatitis
  • Chronic pancreatitis
  • Pancreatic cyst
  • Pancreatic tumor/cancer

Gallbladder & Bile Duct Problems

  • Small gallstones/sludge
  • CBD stones
  • Obstructive jaundice

Cancer Evaluation

  • Staging of GI cancers
  • Checking lymph node spread
  • Detecting small tumors

Unexplained Symptoms

  • Persistent abdominal pain
  • Weight loss
  • Recurrent vomiting
  • Unexplained jaundice

Abnormal Imaging Reports

If CT/MRI/USG shows:

  • Lump
  • Thickening
  • Enlarged lymph nodes
  • Suspicious lesion

Types of EUS

Upper EUS

Scope enters through the mouth.

Used for:

  • Esophagus
  • Stomach
  • Pancreas
  • Gallbladder
  • Bile duct

Lower EUS

Scope enters through anus.

Used for:

  • Rectum
  • Colon
  • Anal sphincter
  • Rectal tumors

How Is the Procedure Performed?

Before Procedure

Usually:

  • Fasting for 6–8 hours
  • Certain medicines may need adjustment:
    • Blood thinners
    • Diabetes medications
  • Consent taken
  • IV line inserted

For lower EUS:

  • Bowel preparation may be required

During the Procedure

Step-by-Step
  1. Patient lies on left side
  2. Sedation/anesthesia is given
  3. Mouth guard placed (for upper EUS)
  4. Flexible echoendoscope inserted
  5. Ultrasound images appear on monitor
  6. Doctor examines organs in detail
  7. If needed, biopsy/FNA taken using a needle

The procedure generally takes:

  • 30–60 minutes
  • Longer if biopsy/intervention done

Is EUS Painful?

Usually NO.

Most patients are sedated and:

  • Sleep during procedure
  • Feel minimal discomfort
  • Often remember very little afterward

Some may experience:

  • Mild sore throat
  • Bloating/gas
  • Sleepiness for few hours

These symptoms usually settle within 24 hours.

What is EUS-Guided Biopsy (FNA/FNB)?

Sometimes doctor sees:

  • Mass
  • Lymph node
  • Cyst
  • Suspicious lesion

A thin needle is passed through the scope to collect tissue/fluid sample.

This is called:

  • FNA → Fine Needle Aspiration
  • FNB → Fine Needle Biopsy

It helps confirm:

  • Cancer
  • Infection
  • Inflammation
  • Pancreatitis

Advantages of EUS

Very High Accuracy

Especially for:

  • Pancreas
  • Small tumors
  • CBD stones

Detects Very Small Lesions

Sometimes missed on:

  • CT scan
  • MRI
  • Normal ultrasound

Allows Biopsy

Diagnosis and tissue sampling can happen in same sitting.

Minimally Invasive

No external cuts/surgery.


Risks & Complications

EUS is generally very safe, but rare complications include:

  • Bleeding
  • Infection
  • Sedation reaction
  • Pancreatitis (rare after pancreatic biopsy)
  • Perforation/tear (very rare)

Recovery After EUS

  • Observation for 1–2 hours
  • Soft diet after recovery
  • No driving same day
  • Most people return to normal activity next day

When Should You Contact Doctor After EUS?

Immediately seek help if:

  • Severe abdominal pain
  • Fever
  • Vomiting
  • Black stools
  • Breathing difficulty
  • Heavy bleeding
